We are a multi-disciplinary center, specializing in treating the whole person, not just the diagnosis with the use of traditional and biomedical treatments, in a loving and caring environment.
What you will need for your first visit:
- Please download and complete New Patient Packet and Office Policy
- Please download and review Notice Privacy Practices
- If applicable bring copies of early intervention reports, school reports, consultations, and or medical records
- Bring any prescriptions or supplements you may be taking
Financial Policy
The Center is a direct-pay service practice. The services may be covered under your out-of- network benefits coverage. You can contact your insurance company directly prior to the appointment if you choose to submit out-of- network benefits. An itemized invoice will be provided at the end of your visit for submission to your insurance company, if you choose to do so.
Payments can be made by check, cash, or credit cards at the time of services rendered.
Cancellation Policy
Please call at least 72 hours in advance to cancel an appointment, a $50 fee will be charged if proper notification is not given.
